My published "Normal and Customary Fee" is $165.00 per 50-minute session. While some uninsured patients can afford this, many cannot. (see below)
I am a credentialed provider with several insurance companies, among them are, Anthem, Blue Cross/Blue Shield, Aetna, Cigna, HealthNet, United Behavioral Health, Optum, Magellan and Mail Handlers Benefit.
My services may be covered in full or in part by your health insurance or employee benefit plan. Beginning 1/1/2016, I charge a default copayment of $35.00 unless your insurance company has informed you that your copayment is different. Please check your coverage carefully by asking your insurance company the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the amount due (copayment) per therapy session?
Reduced fee services are available - you and I will discuss this prior to the beginning of treatment to make sure there are no surprises or misunderstandings.

I do not carry balances. Payment (or your contracted portion) is due at the time of service. If your insurace does not reimburse me within 30 days of my submission of the claim, you will be responsible for the the contracted rate (less any copayment you've made already).
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance (via phone call, email or text message) you will be required to pay the fee for a "failed appointment without sufficient notice" - the fee is $85.00.
ADHD (Attention-Deficit/Hyperactivity Disorder)
I provide thorough assessments for adolescents suspected of having ADHD. Usually, insurance companies DO NOT pay for ADHD evaluations. The fee for an ADHD assessment, including reports from the most current Conners-3 scales and summary consultation is $200.00. Please call me to discuss my assessment service.
Importantly associated with ADHD is the brain's "executive function". I can provide executive function assessments using the "Barkley Deficits in Executive Functioning". This scale is appropriate for those 18 years of age and older. The fee charged for the Barkley assessment is $200.00
Pre-Bariatric Proceedure Clearance
Many patients preparing for bariatric surgery are required to have a psychological evaluation prior to the proceedure. My fee for an evaluation and a report sent to your surgeon is $185.00 due at the time of the appointment.
If (a) you are covered by an insurance company that is contracted with me and (b) you have a diagnosable mental condition, your insurance will pay for a PORTION of the evaluation. You will still be responsible for the usual copayment plus $110.00 for preparation and sending the report. (I fax it immediatly after I complete it and mail a hard copy the next business day.) From the date of the evaluation appointment until I fax the report is usually less than seven business days.
